Quote of Benefits

Before your first visit, we will request a quote of benefits from your insurance provider to give you an estimate of your coverage. Please note that this information is based on what your insurance provides at the time and is not a guarantee of coverage.

Changes in Coverage

If your insurance policy or provider changes, please inform our front office staff as soon as possible to prevent any interruptions to your treatment. This helps ensure we have the most accurate and up-to-date information.

Deductible

Your deductible is the amount you must pay out of pocket before your insurance coverage begins. Once this amount is met, your insurance will start to contribute to your care.

Co-Insurance

Co-insurance is the percentage of costs you’re responsible for after your deductible is met. The amount you owe can vary depending on the service and the allowed amount for that claim.

Copay

Your copay is a fixed amount you pay for a service, regardless of the total cost of the claim. This amount usually stays the same for each visit or service.
